Health Disparities Profiles - US Department of Health and Human Services
State-by-state reports from 2011 show causes of death, health risk factors, and other health statistics broken down by race. National and state maps show where greatest disparities lie.

Health of Houston 2010 - The University of Texas School of Public Health & Institute for Health Policy
A population survey on health-related data for Houston and Harris County residents. Includes a report and interactive maps.

National Healthcare Quality & Disparities Reports - Agency for Healthcare Research & Quality, National Institute of Health
